Eat and Learn Diabete Conversational Map Programme

The ‘Eat & Learn with Diabetes Conversation Map’ program is an enhance innovative education method that added with practical interactive activities. You will have ample opportunities learning from peers & healthcare professionals, sharing your experience and learning new strategies in managing diabetes.

This is a 3-session, 2 ½ hours per session program which is conducted over 3 consecutive weeks. Each session consists of 6 – 10 participants. A trained Diabetes Educator will facilitate the discussion in a round table setting using the Diabetes Conversation Maptm education tools.

Session 1 : Living with Diabetes & How Diabetes Works
Session 2 : Healthy Eating & Being Active
Session 3 : Starting Insulin Therapy
  1. A person with diabetes
  2. Able to speak and read English
  3. Priority will be given to SIme Darby Medical Centre patients
